Your Choices

You can tell us your choices about:

  • Sharing information with your family, close friends, or others involved in your care

  • Including your information in a patient directory

  • Marketing communications or fundraising

If you cannot tell us your preferences (e.g., if you are unconscious), we may go ahead and share your information if we believe it is in your best interest.

Our Uses and Disclosures

We typically use or share your health information in the following ways:

1. To Treat You

We can use your health information and share it with other professionals who are treating you.
Example: Colleen Graziano, APRN-BC, may consult with other providers as part of your telehealth care plan.

2. To Run Our Organization

We can use and share your health information to run our practice, improve your care, and contact you when necessary.
Example: We may use your information to assess the quality of care or to contact you for appointment reminders.

3. To Bill for Services

We can use and share your health information to bill and get payment from health plans or other entities.
Example: We give your health plan information about a treatment you received so it will pay for your services.

Other Ways We May Use or Share Your Information

We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as:

  • Public health and safety (e.g., reporting adverse reactions, preventing disease)

  • Research purposes

  • Compliance with the law (e.g., sharing with HHS if required)

  • Law enforcement or legal proceedings

  • Responding to workers’ compensation, law enforcement, and government requests

We will never share your information for marketing purposes, sell your information, or share psychotherapy notes without your written permission.
